ADEii Health Foundation is a nonprofit organization that designs and advances community-based healthcare infrastructure. We bring together community organizations, public agencies, clinicians, researchers, and funders to develop practical models that move care out of isolated systems and into the places people already know and trust.
Care Before Crisis is not simply a message. It is a systems-design principle.
OUR CHALLENGE: The problem is not a lack of effort. It is disconnected infrastructure.
Healthcare, behavioral health, housing, education, employment, and economic stability are deeply connected. Yet the systems responsible for addressing these needs often operate separately.
People are expected to navigate multiple providers, eligibility requirements, referral processes, and administrative systems—frequently at times when they have the least time or capacity to do so.
Meanwhile, trusted community organizations are often the first to recognize when someone needs help. These organizations have relationships, local knowledge, and credibility, but may lack the clinical and operational infrastructure required to create sustainable pathways to care.
The result is a familiar pattern:
Support arrives too late.
Referrals do not become completed connections.
Community organizations are asked to solve problems without adequate infrastructure.
Workforce shortages limit access.
Short-term programs disappear when initial funding ends.
People fall through the spaces between systems.
OUR THESIS: Care should be built around trust.
We believe better infrastructure can change that.
Access improves when the healthcare infrastructure moves closer to the relationships people already trust. Instead of requiring every person to find and enter a separate clinical system, we explore ways to connect care with the places people already live, learn, work, and seek support. The goal is not to turn every community organization into a healthcare provider.
The goal is to give trusted organizations access to the partnerships, systems, and infrastructure needed to create responsible pathways to care.
THE LAB: From insight to infrastructure
ADEii Health Foundation operates at the intersection of research, systems design, community partnership, and implementation. Community organizations should not have to independently build every component required to support whole-person care or become healthcare organizations, which would pull them away from their missions, create creep and dilute community expertise. Our work follows four connected stages.
01 — Investigate by exploring with the community
We begin with lived realities, trusted relationships, and the knowledge of people already doing the work. We listen to the people closest to the problem, including community organizations, practitioners, public agencies, and residents. We then co-identify structural barriers affecting access, continuity, workforce capacity, coordination, and sustainability. Partner organizations retain the relationships, identity, and community expertise they have already built. ADEii helps develop the connective infrastructure around them.
02 — Design for implementation and build for continuity
An idea is only useful if organizations can operate it responsibly in real conditions. We develop practical models that connect community trust with clinical, operational, and technological infrastructure. Every model begins with a real setting, a defined need, and a clear path toward implementation. ADEii has partnered with state agencies and local organizations to:
Develop supervised training-to-career pathways that expand workforce capacity while creating meaningful opportunities for emerging behavioral health professionals. The objective is a workforce that is prepared through real-world, community-rooted experience.
Explore systems that help people understand their options, complete referrals, navigate benefits and coverage, and remain connected during periods of change. A referral is not an outcome. The work continues until a meaningful connection is made.
Design shared clinical and operational systems that can support multiple community locations without requiring every organization to build an independent healthcare operation. This includes examining lower-fixed-cost models, flexible deployment, shared quality systems, and sustainable reimbursement pathways.
03 —Deploy to share infrastructure
Together, we measure what matters. We look beyond activity counts to examine access, completed connections, continuity, experience, and long-term capacity. We document lessons, measure outcomes, strengthen operating systems, and prepare effective models for broader adoption. We work with aligned organizations to test ideas in real community environments. Deployment allows us to learn what works, what breaks, and what must change before a model can grow. The objective is not innovation for its own sake. It is durable infrastructure that communities can use.
And when philanthropic funding is available, it should create more than a temporary program. Whenever possible, it should help establish infrastructure that can continue beyond the initial investment.
04 — Scale for long-term sustainability
ADEii Health Foundation uses charitable and mission-aligned resources to move promising ideas from concept to implementation—and from implementation to lasting impact. By focusing on reducing costs, eliminating redundancies, refocusing efforts and realigning resources, we strengthen the partnerships, systems, evidence, and funding pathways needed for effective models to endure, adapt, and grow.
OUR INSIGHTS: Ideas should be shared and collaboration works
ADEii Health Foundation is committed to turning field experience into practical knowledge. We are interested in what works, what does not, and what the field can learn from both. The challenges facing healthcare cannot be solved by one organization or sector. We collaborate with partners who are ready to investigate difficult problems, test practical ideas, and build infrastructure that can last.
Community organizations are the ideal partners for implementation, demonstrations and learning.
Public agencies are already exploring models that connect community access to accountable clinical and operational infrastructure.
Funders and foundations can support research, model development, demonstration projects, evaluation and catalytic implementation.
Researchers and evaluators can help develop evidence, examine outcomes and translate field experience into useful knowledge.
Practitioners can contribute clinical, operational and community insight to refine what works and design new models when needed.
Educational institutions have the established expertise and emergent talent to develop training, supervision, research, and workforce development needed to own the thesis and refine the work as people’s needs evolve.

ADEii Health Foundation leads charitable research, systems design, workforce initiatives, partnership development, and catalytic program support. The Foundation brings together research, systems design, partnership development, and catalytic resources to help promising ideas become practical, sustainable models.
ADEii Health, Inc. is a separate clinical and operational organization that may provide behavioral health care, case management, care coordination, and related services through appropriate agreements and reimbursement arrangements.
The ADEii Health Foundation is an independently governed California nonprofit organization. Charitable funds are used exclusively for public-benefit purposes consistent with its nonprofit mission.
